The belt is Belt of the Staff Ones in accessories. The black and white is an event item from fight4freedom. I think one of the deathless npcs had it.

IDK. It looks like it'd be a donation or event item so I would check in those two parts of the inventory. If u mean the f4f item, it's definitely event, and they should be sorted by date available so it'd be between the masquarade items and the nocturne items for this year.

Ah, that sash. As Espy said in the Item ID Thread, it's Zephyr's Companion. You currently have it equipped in blue though. It's under 'special' if you still can't find it ^^;
