@Tina---as much as i hate to say this, you still need to fill out the form since once you have adopted, i tend to delete the form in front... i ask for this form so i could send it to your adoptee so he/she will have an idea about you before he/she contacts you... i apologize for the inconvenience... i'm still thinking of a way as to how frequent parents won't need to repeatedly fill out the form coz i know it is a hassle on your part...
