It's kinda voodoo and kinda not, funny little fusion huh. I'll edit it in here in a sec when i find my version; i've just discovered the Pocket Book is at my boyfriend's house.
And people think that about a lot of things, but the world's not as black and white as they like to think.
Edit: Ok then, found it, my personal little gem of a spell. Obviously something like this isn't to be used without a damn good reason, rule of three and all, so be careful not to do it in malice; hence why my old employer hasn't had a dose of this.

Ashes to Ashes
(so called because at the end you have ash ready to use when it is required)
I found this spell originally in my copy of “The Little Book of Pocket Spells” and tweaked it both for a special occasion (a friend was in need and the arsehole really deserved a psychic bitch-slap) and because I don't like taking spells straight from books/other people without adding something of myself to them.
You will need to cast a full circle for this spell to protect yourself. As with all banishing it's best done during the waning moon but emergencies can't wait obviously, and also performing this on a Saturday would empower it further as Saturn and banishings work well together.
You will need to gather together the following:
Motar & Pestle
Black peppercorns (quantity as needed, depending on the number of names)
Benzoin resin
2 black candles & candlesticks
Plain paper and a black pen
A fireproof bowl
Matches/Lighter/Dragon/etc . . .
Dragon's Blood incense (to empower your spell)
A small black pouch (not voile as you'll be putting ash in here)
Something comfy to sit on (you'll be here a while)
Gather everything you'll need into one place within easy reach and then cast your circle however you like. Make sure you have plenty of room to move about! You're gonna need it.
Set up your alter within the circle (mine was a simple candles-chalice-crystal-incense deal) and light the Dragon's Blood incense. This will not only empower you and your magick working, but it also doubles as the air part of your alter! Also light the candles.
Now place the peppercorns into the mortar and grind them up. This took me about 20 minutes to do to my satisfaction (you're after a powder). Once that is sorted add some small chunks of the benzoin resin (you may need to hit it with a toffee hammer to get small enough bits) and grind and mix them into the peppercorn powder. Tap some of the ash from the incense into this and mix. You'll be left with a banishment powder with a smokey, peppery smell that has a slightly sweet undertone. It's very strong smelling so don't breathe it in too deeply!
Putting that to one side for the moment, take the pen and paper and write the name/s of whoever your spell is aimed at, then fold the paper so that there is a crease through the writing. Fold again and then scrunch up (easier to catch light later), all the while imagining their power diminishing and their hold weakening.
Place the paper in the bowl and using the flame of one of the candles (either the thing itself or using a stick or taper – decidedly less messy) set fire to the paper. Take a tiny pinch of the powder and gently throw it onto the flames. Keep it going until there is nothing but ash.
Once the bowl is cool enough to touch, tip the ash into the mortar and mix it into the powder. You'll be keeping this mixture in case there is any further trouble (doubtful) and for now might like to sprinkle some around/under a plant to bind their energy to the plant.
Tip the remainder of this mixture into the black pouch (hence the warning on using voile) and store safely until it is needed, for it can form the base of many more banishment spells to personalise them to this individual.
Have a shower and eat some chocolate afterwards. Trust me you'll need it, but boy is this spell worth it.

Trust me, the choccies were well needed, and there's a lot of herbal work because i like working with them. As i've done, take it, use it, change it around, substitute, add and remove what you want. When you make a spell your own, that's when it comes alive. That's when you perform your best magick.
And as you can see from the "dragon" comment in the items list, i put humour into my BoS.
